- Measure the abdomen at the level of the navel (belly button). Round down to the nearest 0.50 inch, repeat the measurement three times, and average these measurements. Write this average down.
- Enter the average neck circumference from step 3 and the average abdominal circumference from step 4 into the respective fields.
- Calculate the circumference value by subtracting the average neck circumference (step 3) from the average abdominal circumference (step 4) and enter this value.
- Record the height in inches and ensure it is to the nearest 0.50 inch. Now, find the soldier's circumference value (step 6) and height (step 7) in figure B-1 to determine the percent body fat.
- Enter the calculated percent body fat value into the appropriate field based on the data obtained from figure B-1.
- Indicate compliance with Army standards by checking the appropriate box. Note any recommendations regarding weight loss if the soldier is not in compliance.
- Finalize the form by obtaining the signature of the person who prepared the form, along with their rank and date.
- Make sure the supervisor checks and approves the form with their printed name, signature, rank, and date.
