- Finally, fill out Part V, 'Rated Officer Contributions'. Enter the date, height, and weight for the APFT section. Additionally, list your significant contributions, any unique professional skills or areas of expertise valuable to the Army, your preferred branch or career field should you be unable to serve in your current one, and three future assignments for which you feel best suited. Don’t forget to add your signature and date.
