Individuals completing the Shipper's Declaration for Dangerous Goods form must provide the full name, address, and telephone number of the receiver/consignee. The Air Waybill or air consignment note is a receipt issued by an international airline for all shipments of goods by air.

I hereby declare that the contents of this consignment are fully and accurately described above by the proper shipping name, and are classified, packaged marked and labelled/placarded, and are in all respects in proper condition for transport ing to applicable international and national governmental regulations.

A shipper's declaration of dangerous goods is not required unless the dry ice is used as a refrigerant for other dangerous goods. Otherwise only an air waybill is required with the following information:Dry Ice or Carbon dioxide, solid, 9, UN 1845, number of packages and the net quantity of dry ice in each package.

Note: Never use whiteout. Always make changes by striking out incorrect information, carefully adding the correct information and then signing the correction (no initials). The shipper's declaration must always be filled out completely.

If you're transporting dangerous goods by ocean freight, you must complete a shipper's declaration ing to the International Maritime Dangerous Goods Regulations Code (IMDG Code). The IMO endorses the regulations to ensure safety during transportation by ocean vessels.

It must be the same person that is signing the declaration. The Importer's Declaration of U.S. Goods Returned (Importer's Declaration) can be completed by the owner, importer, consignee, or agent having knowledge of the facts regarding the claim for free entry.
