10. Complete the submitter information by printing the name, date, telephone number, and identifying whether the submitter is the owner, builder/contractor, insurance agent, engineer, or another specified role.
11. After completing all sections, review your entries for accuracy. You can then save your changes, download a copy, print the form, or share it as needed.
