Follow the steps to fill out the Workplace Action Plan online.

1. Click ‘Get Form’ button to access the Workplace Action Plan and open it in your preferred electronic format.
2. Begin by answering each checklist question with ‘yes’ or ‘no’. For every ‘no’ answer, provide a risk rating using the provided matrix and enter further explanations in the comments section.
3. Once the inspection is complete, transfer the ‘yes/no’ responses to your electronic copy of the Workplace Action Plan. Complete the scoring and action list as instructed.
4. Score each section by allocating a score of ‘1’ for each ‘yes’ answer and a score of ‘0’ for each ‘no’. At the end of the checklist, calculate the total and present it as a percentage.
5. Add all ‘no’ responses to the ‘Action List’ and propose appropriate actions for rectification as necessary.
6. To save your completed form, select ‘File’, then choose ‘Print’. In the printer drop-down menu, select ‘Adobe PDF’ as your printer.
7. Adjust the orientation to ‘Landscape’ before proceeding to print.
8. Select ‘Print’, which will allow you to save the Workplace Action Plan as a PDF file in a location of your choice.
9. Finally, email the completed PDF to the designated contact at Konekt (OHSassistance@konekt.com.au) to ensure data entry into the central risk register. Be sure to copy your employing Senator or Member on this email.
