- Choose the remittance type by marking either 'Normal' or 'Settled in Advance'. If applicable, provide remarks in the specified limit of 22 characters.
- If applicable, include the intermediary bank information and the ID number of the receiver for settled in advance transactions.
- Complete the form by entering the date, city, and your signature(s). Remember to submit the form along with a copy of your ID.
