How to fill out the Utah Ownership Statement online

The Utah Ownership Statement is a critical document used for declaring ownership of a vehicle in the state of Utah. This guide will provide you with clear, step-by-step instructions on how to accurately complete the form online.

Follow the steps to fill out the Utah Ownership Statement online

  1. Click ‘Get Form’ button to access the Utah Ownership Statement and open it in the editor.
  2. Begin by filling in the owner information section. Here, you will need to provide the primary owner's name, including their last, first, and middle initial. Ensure this is entered accurately.
  3. Next, fill out the address information for the primary owner, including the street address, city, state, and ZIP code. If there is a secondary owner, include their details in the corresponding section.
  4. Enter information regarding the business name, if applicable, and provide the Federal Employer Identification Number (FEIN). If the vehicle is part of a fleet, include the fleet number.
  5. Complete the section for the Vehicle Identification Number (VIN) or Hull Identification Number (HIN), followed by the year, make, model, and state last registered. Include the license number and ensure all details match the vehicle documentation.
  6. Disclose the odometer reading. Here, you must indicate the actual mileage without tenths. If applicable, indicate if there is an odometer discrepancy or if the mileage exceeds mechanical limits.
  7. Provide the date when the vehicle was possessed and the name and address of the applicant, ensuring to include the city, state, and ZIP code.
  8. State the fair market value of the vehicle. Remember, if this amount exceeds $1,000, you might be required to submit a surety bond.
  9. Complete the facts and indemnification agreement by providing the name or company from whom the vehicle was acquired, along with their address.
  10. Explain in detail why you did not obtain an outstanding certificate of title or why the attached title is not negotiable. This explanation should include how you acquired the vehicle, parties involved, the acquisition date, and its working condition.
  11. Sign and date the form, affirming under penalties of perjury that the information provided is true and correct.
  12. Finally, review your completed form for accuracy. After ensuring everything is correct, you can save your changes, download, print, or share the document as needed.

It is unwise to rely on a seller's promise to “get the title to you.” Although Utah law requires a seller to provide a title within 48 hours of the transaction, a common-sense approach is to obtain a title at the time of sale, or wait to pay for the vehicle until the seller can provide the title.

These states are Arizona, California, Colorado, Florida, Georgia, Hawaii, Idaho, Kansas, Louisiana, Massachusetts, New York, North Carolina, Ohio, Pennsylvania, South Carolina, Texas, Utah, Virginia, Washington, and Wisconsin.

Examples of "ownership evidence" can be a bill of sale, power of attorney, receipt, canceled check or money order, temporary registration/maker, affidavit of legal possession or endorsed copy of at title.

Vehicles – Every vehicle (including motor vehicles, motorcycles, trailers, semi-trailers, and manufactured homes) operated in Utah, regardless of model year, must be titled except non-commercial trailers that have an unladen weight of 750 pounds or less.

You may request a duplicate title online using our Motor Vehicle Portal (MVP), or by completing Form TC-123, Application for Duplicate Utah Title. A $6.00 title fee will be assessed, and the duplicate title should be received in about a week after making the application.

Most websites list Utah as a Title Theory state. In a Title State, the lender holds title to the property in the name of the borrower through a Deed of Trust. When the loan is completely paid off, the lender issues and records a Deed of Reconveyance in favor of the borrower who then has clear title to the property.

Replacing/Duplicates by Mail You will file the form listed above, write a check or money order for the fees, and send your documents to the Utah DMV. You should receive your new title in the mail within a week.
