3. In the business details section, provide your business name, indicate whether you are an existing pet shop operator, and include your Australian Business Number (ABN) along with the business street address, PO Box number, suburb, and postcode.
4. Next, specify the premises you intend to use as a fauna dealer. For the first premises, enter the street address (or indicate 'as above'), PO Box number, suburb, and postcode. If you need a registration certificate for additional premises, refer to Attachment 2 and note that a fee is applicable for each extra premises.
5. Provide the required supporting evidence in Attachment 1, detailing your knowledge and skills in captive animal management and reptile husbandry. Ensure to list any relevant licences or permits you have held.
6. Review the fee schedule. Note that the fauna dealer licence fee is $2400 for trading reptiles, which covers registration for one premises. Additional premises incur extra fees.
7. Complete the payment section by entering your credit card details. All fields must be completed, including credit card type, expiry date, number, CVV, and the name on the card. Provide your signature to authorize the transaction.
8. In the declaration section, affirm that all information provided is true and correct and sign with the date. Make sure to review and comply with the requirements outlined by the Office of Environment and Heritage.
9. After completing the form, review all sections for accuracy. Save your changes, then download, print, or share the completed form as needed.
