- Complete Section C by obtaining the necessary signatures before the absence. This section requires the student's signature, a parent’s signature, and the director's signature, along with the corresponding dates for each.
- If any daily work was not completed upon return, ensure to sign Section D, which also requires a parent's signature and date.
- Finally, if any tests or quizzes were not made up by the due date, sign Section E, including the parent's signature and date.
