Follow the steps to successfully complete the Planned Absence Form.

2. Begin with Section A, where you will need to enter the student's name, their homeroom, the reason for the absence, and the duration of the absence, including the beginning and ending date/time. Additionally, specify the date when any tests or quizzes are due, keeping in mind that all daily work is due the day following the absence.
3. Proceed to Section B. Here, you will list all daily work, tests, and quizzes that the student will miss during their absence. If more space is needed, attach an additional sheet and indicate 'SEE ATTACHED'. If no work will be missed, simply write 'NONE'. Upon return from the absence, indicate whether the daily work and tests/quizzes have been completed by circling 'YES' or 'NO' and ensure the teacher's initials are provided.
4. Complete Section C by obtaining the necessary signatures before the absence. This section requires the student's signature, a parent’s signature, and the director's signature, along with the corresponding dates for each.
5. If any daily work was not completed upon return, ensure to sign Section D, which also requires a parent's signature and date.
6. Finally, if any tests or quizzes were not made up by the due date, sign Section E, including the parent's signature and date.
7. After filling out all sections, review the document for accuracy. You can then save your changes, download the completed form, print it, or share it as needed.
