10. Compose your PERSONAL STATEMENT, providing additional information that supports your application. You may attach additional pages if necessary.
11. Answer the INFORMATION question regarding how you learned about UCD and the programme you wish to study.
12. In the DECLARATION section, confirm that all the information is true, complete, and accurate. Ensure you sign and date the form.
