- Remember, the college waives fees for full-time employees/retirees for up to 12 credit hours and for their spouse and dependents for up to 15 credit hours. Review any additional fees that may apply beyond this allowance.
- Once you have filled all necessary sections, ensure to include the date and any required signatures from your supervisor, chancellor, or designee, if applicable. Note that a signature is not necessary for spouse or dependent requests.
- After completing the form, you can choose to save your changes, download a copy for your records, print it out for submission, or share it with relevant departments.
