10. Provide information concerning the country of origin, using the correct terminology for where the specimens were taken or bred.
11. Complete any relevant details for re-exports, including the country of last re-export and corresponding certificate numbers.
12. Finalize the document by ensuring all information is accurate. Save your changes and then download or print the completed form for submission.
