- Denote the appendix number and the source of the specimens as outlined in the guidelines provided.
- Provide information concerning the country of origin, using the correct terminology for where the specimens were taken or bred.
- Complete any relevant details for re-exports, including the country of last re-export and corresponding certificate numbers.
- Finalize the document by ensuring all information is accurate. Save your changes and then download or print the completed form for submission.
