9. Proceed to the nomination details. You can list up to three nominees. If a nominee is a minor, provide their date of birth and specify a guardian's details.
10. For each nominee, enter the percentage share of your accumulated funds that each will receive in the event of your passing. Ensure that the total equals 100%.
11. Read the declaration section carefully, then sign or provide your thumb impression in the designated box. Ensure it is within the box to avoid rejection.
12. The final step is for the aggregator to complete their authorization portion. Ensure they fill out the required details and affix their signature and stamp.
