- In Section IV - Applicant’s Test Status, provide details regarding your training status, including whether you have completed the Indiana 105 hour Training, are a foreign nurse, transferring from another school, or are currently a nursing student. Additionally, indicate if you are a psychiatric attendant or a graduate nurse awaiting board exams.
- Complete Section V - Test/Monitor Information. For each test you have taken, input the test entity, monitor, site, and test date. Indicate whether you passed or failed the written, oral, and skills tests for each of the tests listed.
- Conclude by reviewing all entered information for accuracy. Make any necessary corrections before finalizing. Once everything is done, you can save your changes, download, print, or share the form as needed.
