- Move to Section C and provide your visa immigration activity. List the date you entered the U.S., the start and end dates of your current purpose or activity, and your complete U.S. immigration history since January 1, 1988.
- In Section D, indicate your residence status for tax purposes. Select the option that applies to you and provide any necessary details as requested.
- If you are expecting honorarium payments, complete Section E by answering the questions about the nature and duration of the activity.
- Finally, certify that all information is true and correct. Provide your signature and the date to validate the form.
- Once all sections are completed, save your changes and choose to download, print, or share the form as needed.
