Follow the steps to successfully complete the Learning Progress Form

1. Press the ‘Get Form’ button to access the Learning Progress Form and open it for completion.
2. Enter the child's name in the designated fields, including last and first names.
3. Fill in the child’s date of birth accurately in the specified format.
4. Select the elementary school where the child will attend kindergarten from the dropdown list or by using the school locator provided.
5. Indicate whether the child has an Individualized Education Plan (IEP) or an Individual Family Service Plan (IFSP) by checking the appropriate box.
6. Provide names for both Parent/Guardian #1 and Parent/Guardian #2, including last and first names.
7. List the language(s) spoken at home by the child.
8. Identify the individual who completed the form by entering their name.
9. Date the completion of the form to maintain accurate records.
10. Fill out the program name, phone number, address, city, state, zip code, and type of program the child is attending.
11. Confirm whether a parental consent form has been signed to release this information, choosing 'Yes' or 'No' as appropriate.
12. Check all applicable learning styles preferences, including play preferences indicated on the form.
13. Assess the child's progress in each learning domain by selecting 'Satisfactory' or 'Needs Support', and provide additional comments where necessary.
14. Review the completed form for accuracy before proceeding to save changes.
15. Once all fields are completed, save, download, or print the form for submission to the Early Childhood Programs at HCPSS.
