- Indicate any others who should be given notice of your application. Provide their full names, relationship to the children listed, and any necessary contact details.
- If you have a solicitor, complete the solicitor's details section with their name, firm, address, and contact information. If you do not have a solicitor, review section R of leaflet CB1.
- Provide brief details about what you are applying for and the reasons for your application in the details of application section.
- In the attending the court section, if an interpreter is needed, specify the language required. Also, indicate if there are any special arrangements needed due to a disability.
- Complete the statement of truth section by selecting the appropriate options, printing your name, signing, and dating the document.
- Before submitting, check that all necessary documents are attached, including any required fees, and that you have completed and signed Section 8.
- Finally, save your changes, download, print, or share the form as needed.
