9. In the attending the court section, if an interpreter is needed, specify the language required. Also, indicate if there are any special arrangements needed due to a disability.
10. Complete the statement of truth section by selecting the appropriate options, printing your name, signing, and dating the document.
11. Before submitting, check that all necessary documents are attached, including any required fees, and that you have completed and signed Section 8.
