- If you are completing the form for multiple births, move on to Section D. Provide the names and details of any siblings including date of birth and gender, ensuring the birth certificate PIN numbers are also included.
- Section E requires you to select the student's religion. Tick one box that represents the student’s religious affiliation or specify if it is not listed.
- Finally, complete Section F with the parent or guardian's declaration. Print or sign the name in block letters and provide contact information as required.
- Once all sections are filled out, review the information for accuracy. You can save changes, download, print, or share the completed form as needed.
