- Once completed, send the form to the Santander Nominee Service at Aspect House, Spencer Road, Lancing, West Sussex, BN99 6DA, United Kingdom. Include a cheque for £10 payable to Equiniti Financial Services Limited.
- Keep a copy of the completed form for your records. Check that you have saved any changes before closing the document.
