Follow the steps to efficiently complete the score sheet

1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Select the appropriate class for your dog by checking the box next to Basic, Elementary, Novice, or Advanced.
3. Enter your dog's identification number in the designated field.
4. Specify the height category of your dog by marking the corresponding box: 8”, 12”, 16”, 20”, or 24”.
5. Fill in the breed of your dog in the provided text field.
6. Indicate the age level by checking Junior, Intermediate, or Senior.
7. Record the date of the agility trial in the date field.
8. For each obstacle your dog encounters, indicate any deductions by marking the appropriate boxes next to the listed infractions under each obstacle type.
9. Calculate points off based on your dog's performance by checking the relevant infractions across all sections, including Jumps, Table, and Weave Poles.
10. Compute the sub-total score by tallying up the points deducted from the maximum possible score of 200 points.
11. Record the dog’s time and compare it to the standard course time (SCT) to note any time faults, applying points off for seconds over SCT.
12. Finally, review all sections for accuracy before submitting the form. Save changes, download, print, or share the completed score sheet as needed.
