When is a needle biopsy needed for thyroid nodule?

If the nodule appears suspicious on ultrasound and is larger than 1 cm, the next step is to perform a thyroid biopsy.

Most FNA thyroid biopsies will show that the thyroid nodule is benign, because, most thyroid nodules are benign (non-cancerous). Rarely, the FNA thyroid biopsy may come back as benign even though a diagnosis of a thyroid cancer is actually present.

Fine needle biopsy of a thyroid nodule is indicated in the following situations (5– 7): Patients with clinical signs of thyroid cancer. Nodules >1 cm with at least two ultrasound criteria for malignancy. Nodules of any size with extracapsular extension or indeterminate cervical lymph nodes.
