7. Detail the emergency contacts by listing two individuals who can act on your behalf in an emergency. Provide their names, relationships to your child, and contact numbers.
8. In the 'Session Details' section, highlight your preferred days and session times. Fill in your preferred start date for your child’s attendance.
9. In the 'Application' section, express your wish to apply for a place at the after-school club. Acknowledge your understanding of the policies by checking the appropriate box. Include a non-returnable deposit amount.
10. Finally, sign and date the form where indicated. Ensure that you print your name below your signature for clarity.
