- To submit your request, send a check or money order for $3.00 per official transcript to the MTCC Admissions Office. Ensure all checks include your physical address, phone number, and driver’s license number.
- Alternatively, you can fax the completed form to (828) 659‐0430.
- Fill in the amount for the transcript fees and provide your credit card information if you prefer to pay by card.

Students should contact their high school to request their official transcripts be sent to COTC. Transcripts can be submitted as follows: Mailed to COTC, The Gateway – Student Records, 1179 University Drive, Newark, OH 43055. Emailed directly from high school personnel to cotcrecords@mail.cotc.edu.
