The beneficiary designation forms allow you to name primary and secondary beneficiaries. Your “primary beneficiaries” are the first people or entities that you want to receive your benefit after you die.

Change in Beneficiary The member may change his or her beneficiary(ies) by filing a new Form 6.11, “Designation of Beneficiary.” IMRF recommends members use their online Member Access account to make beneficiary updates. A member who gets married, divorced, or whose spouse dies should file a new Form 6.11.

Change a beneficiary Generally, you can review and update your beneficiary designations by contacting the company or organization that provides your insurance or retirement plan. You can sometimes do this online. Otherwise, you'll have to complete, sign, and mail a paper form.

How can I contact IMRF? The best way to contact us is through Member Access. You can also call 1-800-ASK-IMRF (1-800-275-4673), email us or fax us at 630-706-4289. We strongly recommend that you use your Member Access account to manage your IMRF benefits.

You will see your IMRF Member ID Number on your member statement in place of your Social Security Number. You will also see your IMRF Member ID Number on any correspondence you receive from IMRF, and you can use it when calling IMRF or when completing an IMRF form.

Once You've Named Your Beneficiaries, Keep Them Current Whenever you open a financial account, naming a beneficiary should be one of your top priorities. But as your life changes, it's important to make sure you update the beneficiary information on all of your accounts.

If name change is due to change in marital status, please attach Form 6.11, “Designation of Beneficiary.” You can download Form 6.11 from our website or update your beneficiaries online through Member Access at .imrf.org • Please file a copy of this form with your employer.

Write only one beneficiary on each line. Make sure that you write the full names of all beneficiaries. For example, if you name you children as beneficiaries, DO NOT merely write “children” on one of the lines; instead write the full names of each of your children on separate lines.
