- If you ticked ‘2’ in step 5, indicate if the form was received within the opt-out period based on enrollment or confirmation date by ticking yes or no, and include the later date.
- If the response to step 7 was ‘no’, specify if the form was returned due to incorrect completion, which would extend the opt-out period. Tick ‘yes’ or ‘no’ accordingly.
- For any ‘yes’ responses in steps 6, 7, or 8, arrange to refund contributions locally; for ‘no’ responses, note that the HSC Pension Service will issue refunds upon receipt of claim form REF1.
