This guide provides clear and supportive instructions on how to complete the Pakmission form online for the NICOP application. Our aim is to assist users through each section to ensure a successful submission.

Follow the steps to fill out the NICOP application form online.

1. Press the ‘Get Form’ button to access the Pakmission online form and open it in your preferred editor.
2. Select the type of application by checking the appropriate box in Line 1.
3. On Line 2, input the MNIC/CNIC/NICOP number of the family head and specify the relationship.
4. For Lines 4-5, write the applicant’s name and their respective MNIC/CNIC/NICOP number. If the applicant is a minor born in Pakistan, include their CRC/Bay-Form number if applicable.
5. Complete Lines 7 to 22 by providing all required personal information accurately. Be cautious, as any inaccuracies may lead to rejection of your application.
6. In Line 25, indicate the status if applicable to a Jammu & Kashmir citizen.
7. Fill in Lines 26 with your home and cell phone numbers, and provide a clear email address.
8. List your blood group in Line 27.
9. In Line 28, ensure to check the appropriate box. Note that both left thumb impression and signature are required. A parent can sign on behalf of the applicant if necessary.
10. Complete Lines 29, 30 & 31 with information of the attester, who should not be a relative. Include their signature, name, and CNIC/NICOP number.
11. Line 32 is for official use, leave it blank.
12. Provide blood relative information in Line 33, including names, relationship, and their CNIC/NICOP numbers.
13. Line 34 requires a signature or thumb impression from Muslims, marking N/A for non-Muslims.
14. Specify the home delivery option by checking the relevant box for delivery in Canada, Pakistan, or to the consulate.
15. After completing the form, ensure to save your changes. You can download, print, or share the completed form as needed.
