- Answer whether you would be willing to modify your equipment to operate on MARS frequencies upon acceptance. Indicate if you are willing to be deployed to support the MARS mission.
- Check the modes your station can currently operate on, including any additional types listed under Sound Card Modes and Other.
- Indicate whether you have emergency power capabilities and if you have a mobile station.
- Complete the Privacy Act Statement and agree to the necessary conditions regarding publication of your information.
- Sign and date the application, and if applicable, have the Unit Commander sign if applying for a Military Unit Station.
