- Ask the employee if they would like to donate any annual leave to the Crisis Leave Pool, and if yes, ensure a leave slip is completed.
- At the end of the checklist, both the employee and supervisor must provide their signatures and the corresponding dates before submission.
- Submit the completed and signed checklist to Human Resources or use the SUBMIT button to finalize the process.
