Follow the steps to complete your scholarship application online.

1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Begin with the personal information section. Input your full name, Social Security Number (required for Higher Education Commission records), date of birth, and current residential address. Ensure that your address matches the one registered for voting or listed on your driver’s license. Indicate your marital status and provide both home and cell phone numbers, along with your email address. Include how long you have lived at your current address and the name of the institution you wish to attend.
3. Move to the academic history section. Specify your current level of education, your high school or college graduation status, and the date of graduation. If you are currently attending a college, university, or trade school, please fill in that information. Indicate whether you will attend full-time or part-time, and if part-time, specify the number of credit hours per semester.
4. Fill in your cumulative grade point average for both high school and college. If applicable, provide the date you took the SAT along with your scores in math, writing, and reading.
5. Complete the community involvement section by listing any volunteer work, civic activities, or extracurricular participation. This helps demonstrate your engagement and commitment.
6. In the financial information section, describe any additional financial resources you expect to receive. Indicate your total family income and any special financial circumstances that should be considered.
7. Attach a personal statement of no more than 300 words that outlines your reasons for wanting to attend a college, university, or trade school. Include your fields of interest, post-education ambitions, and needs for financial assistance.
8. Arrange for a letter of reference to be sent to the Annapolis address. The letter should come from someone who knows your educational or work experience, such as a teacher or employer, and must be emailed to anne.healey@house.state.md.us, received by April 1.
9. Finally, verify that you are a resident of Legislative District 22 by signing the application. Include the date and, if applicable, a parent or guardian’s signature.
10. After filling out all sections, save any changes and download or print copies of your completed application and letter of reference. Submit the application along with the letter of reference via email by the April 1 deadline.
