Additionally, provide the Date ORP Participation Code was entered in the payroll system and indicate the Basic Contribution Rate by checking the corresponding box.
  7. Lastly, fill in the Benefits Administrator Information section. The designated Benefits Administrator needs to print their name, sign the document, and provide their telephone number and email address.
