- Calculate your defined taxable payroll by subtracting the amount in item 10 from item 9, and enter this amount in item 11.
- Enter your tax rate in item 12, and calculate your tax due for the quarter by multiplying the taxable payroll from item 11 by the tax rate. Record this in item 13.
- If filing after the due date, compute the interest due in item 14 and add any late filing fees required in item 15.
- Complete items 16 and 17 if you made Electronic Fund Transfer payments or have any credits to record.
- Sum the items appropriately and enter the total amount due in item 18.
- Finally, sign the form in item 19, print your name and title, provide your contact number, and date the report.
