- Complete the slurry data section. Fill out the type of casing and the number of sacks of cement used in each slurry. Include additives, volume, and other relevant slurry information.
- If applicable, report cementing details for plugging and abandonment. Provide data for each plug used, including size, depth, and type of cement.
- Contact information for the cementer’s representative and operator’s representative must be added, including names, titles, signatures, addresses, and phone numbers.
- Finally, review all entries for correctness. Once satisfied, you can save your changes, download, print, or share the completed form.
