THREE TYPES OF NOTIFIABLE INCIDENT Section 35 of the WHS Act defines a 'notifiable incident' as: > the death of a person, or > the serious injury or illness of a person, or > a dangerous incident.

Abuse and allegations of abuse You must notify CQC about abuse or alleged abuse involving a person(s) using your service. This includes where the person(s) is either the victim(s) or the abuser(s), or both.

Incident reports include all of the facts related to the incident, such as the contact information of the people involved; a description of the incident itself; and any follow-up actions that were taken, like medical treatment.

A notifiable safety incident has two statutory definitions, depending on whether the healthcare organisation is an NHS body or not. The organisation has to explain to the patient what's known at the time, what further enquiries will be made, offer an apology and keep a written record of the notification to the patient.

A 'notifiable incident' is: the death of a person. a 'serious injury or illness', or. a 'dangerous incident' that exposes someone to a serious risk, even if no one is injured.

If there is a serious injury or illness, a death or a dangerous incident, you must report it to us immediately on 13 10 50 as an urgent investigation might be needed. Incidents can be notified 24 hours a day, 7 days a week by calling 13 10 50.

A 'notifiable incident' means: the death of a person; or. a serious injury or illness of a person; or. a dangerous incident.
