The Essential Drugs Programme (EDP) is one the pillars of the public health system in South Africa. The National Essential Medicines List Committee (NEMLC) is mandated by the Minister of Health to select essential medicines for the three levels of care, namely primary, secondary and tertiary/quaternary.

The Medicines Management Cycle illustrates the main steps of effective management of medicines supply: selection, quantification and forecasting, procurement, storage, and distribution.

They are selected with due regard to public health relevance, evidence on efficacy and safety, and comparative cost-effectiveness.

The Commission identified five areas that are crucial to essential medicines policies: paying for a basket of essential medicines, making essential medicines affordable, assuring the quality and safety of medicines, promoting quality use of medicines, and developing missing essential medicines.

The NEMLC develops and reviews an essential medicines list (EML) for use in the public sector, accompanied where possible by standard treatment guidelines (STGs). The EML/STGs are prepared for three levels of care, i.e. primary, secondary, and tertiary/quaternary care.

This list contains 1200 recommendations for 591 drugs and 103 therapeutic equivalents. A separate list for children up to 12 years of age, known as the WHO Model List of Essential Medicines for Children (EMLc), was created in 2007 and is in its 9th edition.

Essential medicines are those that satisfy the priority health care needs of the population. They can save lives, reduce suffering and improve health. They are selected based on disease prevalence and public health relevance, evidence of clinical efficacy and safety, and comparative costs and cost-effectiveness.

Various widely accepted criteria are considered in essential medicine selection, such as: disease prevalence. evidence on efficacy. safety. comparative cost-effectiveness.
