1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Begin by filling in the 'Project' field with the name of your proposed project. This should be concise yet descriptive enough to communicate the essence of the project.
3. In the 'Champion' field, provide the full name of the person leading this project. This is important for accountability.
4. Enter the phone number of the project champion in the designated field to allow for easy communication.
5. Select the appropriate focus area for the project by checking one or more boxes: Community Engagement, Military, or Youth/Education.
6. Provide a brief description of the project in the 'Brief Description of Project' section. Clearly outline what activity or event is being proposed.
7. In the 'Who will benefit from this project' section, specify the number of people served and the demographics of the population that will benefit.
8. List any other organizations involved in the project to showcase collaboration.
9. Detail the involvement of Rotary in the project in terms of required funds or volunteers in the respective section.
10. Fill in the 'Time Frame' section, including the start date and completion date of the project.
11. Identify the Rotary leadership team for this project by providing the names of 3-5 Rotarians who will lead or support the project.
12. If funds are required to complete the project, fill out the funds needed in the appropriate section.
13. Indicate the start date and completion date again if funds are being requested.
14. Specify whether you are applying for a matching grant or a simplified grant by checking 'Yes' or 'No'. If applying for a matching grant, include the name of the partner club.
15. Complete the 'Submitted by' field with your name.
16. Sign in the 'Signature' section and provide the date presented to the board.
17. Leave space for the board's decision on approval or decline, as well as for coordinator notification and final report submission date.
18. After completing all sections, you can save your changes, download the filled-out form for your records, and/or print it for submission.
