- Input the quantity ordered in Box 9.
- Select the unit type in Box 10 from the dropdown menu.
- In Box 11, fill in the unit price.
- Once all information is entered, click the Validate Form button to populate totals in Box 12, 13, and 14.

- Print three copies of the form. Retain one for your department and submit the other two copies along with necessary backup documentation to the appropriate office.
