  4. Complete the Authorization for Student Release and Emergency Information Card form (SB45501) for the baby (p. 6), using your baby's details as the 'student.' Ensure all necessary areas are signed.
  5. Gather all completed forms, including the baby’s birth certificate, social security card, and your picture I.D. Submit these documents via fax to (813) 559-8300 or email to Mae.Engram@sdhc.k12.fl.us.
  6. After submitting the documents, the Teen Parent Program personnel will review them. Once approved, a childcare certificate will be generated and sent to both the childcare center and the District's Teen Parent Program.
  7. Complete the 'O' Screen Information form (p. 7) and hand it to the DP clerk at your school. Ensure the enrollment date reflects when the enrollment form was completed.
  8. If you need to transfer your child at any time, contact the Teen Parent Program Office at (813) 272-4674 for assistance.
  9. Upon submitting your paperwork, reach out to your TPP school counselor, Barbara Meshna, via email at Barbara.Meshna@sdhc.k12.fl.us or phone at (813) 272-4805 to schedule a meeting.
  10. Once all forms are filled out and submitted, you can save your progress, download a copy for your records, print the documents, or share them as needed.

Teens are at a higher risk for pregnancy-related high blood pressure (preeclampsia) and its complications than average age mothers. Risks for the baby include premature birth and low birth weight. Preeclampsia can also harm the kidneys or even be fatal for mother or baby.
