The term “prior authorization” may also refer to a commonly used managed care strategy called the “formulary exception process,” which allows exceptions to a plan's formulary (see A M C P 's Concept Series paper, Form u l a ry Management).

NON-FORMULARY EXCEPTION CRITERIA FOR APPROVAL The member must have tried at least three alternative formulary medications that have a similar mechanism of action as the requested medication AND the member either did not respond to or did not tolerate the formulary alternative medications.

An open formulary has no limitation to access to a medication. Open formularies are generally large. A closed formulary is a limited list of medications. A closed formulary may limit drugs to specific physicians, patient care areas, or disease states via formulary restrictions.
