Abdominal aortic aneurysm (AAA) screening is a way of checking if there's a bulge or swelling in the aorta, the main blood vessel that runs from your heart down through your tummy. This bulge or swelling is called an abdominal aortic aneurysm, or AAA.

An AAA doesn't usually pose a serious threat to health, but there's a risk that a larger aneurysm could burst (rupture). A ruptured aneurysm can cause massive internal bleeding, which is usually fatal. Around 8 out of 10 people with a rupture either die before they reach hospital or don't survive surgery.

The average aorta is approximately 2 cm, or slightly smaller than 1 inch. An aneurysm means that the diameter is 1.5 times larger than normal, or 3 cm, in the abdominal aorta. When an aneurysm grows to 5.5 cm in men and 5 cm in women, these tend to be the sizes where surgery is actively considered.

In a recent meta-analysis including four large randomized trials and data from the Medicare and Swedvasc databases, Stather et al1 reported a 14% to 15% mortality at 2 years and 33% to 34% mortality at 4 years or more for elective AAA repair.

Surgery is commonly advised if you develop an AAA larger than 5.5cm in maximum diameter (about 5 cm in women). For these larger aneurysms the risk of rupture is usually higher than the risk of surgery.

A small AAA that gets to be 5.5 cm or larger, or that expands more than 0.5 cm over a six-month period of time, should probably be repaired surgically, if possible. (See 'General treatment principles' above and 'Surgical repair' below.)
