- In section A, provide your CPA Ontario number and your full name. Ensure that your online profile has been reviewed and is current.
- In section B, if you are currently employed and gaining relevant practical experience, indicate this by selecting the appropriate box. You may also want to complete a self-assessment using the recommended tool to confirm your job's qualifications.
- Print your name in the designated field and provide your signature, ensuring to date the form in the format mm/dd/yyyy.
- If applicable, navigate to section C to elect to transition from legacy requirements to CPA Practical Experience Requirements. Read the CA to CPA Student Transition Fact Sheet prior to this section.
- For C1, if you are in a non-approved legacy training program, indicate your election to transition by printing your name, signing, and dating the form.
- For C2, if you are in an approved legacy training program, provide your employer's name, print your name, sign, and also have your Partner/Pre-approved Program Leader do the same, including their signature and date.
- In section C3, if you are starting employment in a newly approved CPA Pre-approved Program, print your name, sign the document, and date it.
- Once all information is completed, review the form for accuracy. You can then save the changes, download the document, print it, or share it as needed.
