  10. Subtract line 5 from line 4 in line 6 to find the subtotal.
  11. If you made monthly deposits, fill out lines 7 and 8 accordingly.
  12. Determine the total tax due on line 9 by subtracting lines 7 and 8 from line 6.
  13. If you incurred a penalty for late filing, include it in line 10 and enter any interest accrued on line 11.
  14. Enter any previous balance or payments received on line 12.
  15. Calculate the balance due on line 13 by summing lines 9 through 12, and ensure it is paid in full with your return.
  16. Sign the form where indicated, provide your daytime phone number and email address.
  17. If a preparer other than yourself is filing, ensure they also sign and provide their information.

A W-4 constitutes legal instructions from employee to employer regarding the number of exemptions and amount of deductions the employer should implement for payroll withholding. There is a flat Nebraska income tax withholding rate of 5% of the winnings from Nebraska that are subject to federal income tax withholding. The amount of income tax withheld must be reported on Federal Form W-2G.
