9. Provide emergency contact information for individuals who are not listed as primary guardians.
10. Fill out any relevant health information or special services that the student may need, circling yes or no where required.
11. Review and sign the parent authorization and indemnity agreement sections, providing your acknowledgment and consent where necessary.
12. Finally, read through the Student/Parent Handbook section and acknowledge its receipt as required. Save your changes once completed.
