- For acting appointments prior to March 27, 2009, fill in the relevant details, similar to the previous step, ensuring to indicate the dates and reasons accordingly.
- Document any retirements or resignations from Post of Responsibility duties, providing the name of the teacher, their PPS Number, the Post Held, Date Left Post, and signature of the Chairperson of BOM.
