The NDI can be scored as a raw scoreor doubled and expressed as a percent. Each section is scored on a 0 to 5 rating scale, in which zero means 'No pain' and 5 means 'Worst imaginable pain'. Points summed to a total score. The test can be interpreted as a raw score, with a maximum score of 50, or as a percentage.

Neck Pain and Disability Scale (NPAD) [1, 2] Item scores range from 0 to 5, and the total score is the sum of the item scores [possible range 0 (no pain)–100 (maximal pain)]. A valid NPAD score can be generated if no more than 15% of the items are missing [10].

The Neck Disability Index is a revised form of the Oswestry Low Back Pain Questionnaire and is designed to measure the activities of daily living in adults with neck pain. It is useful in both clinical practice and in a research setting.
