Fractures of the patella may result from either direct or indirect mechanisms. The classic indirect mechanism is a fall on the feet in which the quadriceps eccentrically fire to decelerate the body. When the force of the fall overwhelms the resistance to knee flexion, the extensor mechanism fails.

Symptoms and Signs of Knee Extensor Mechanism Injuries The affected area is painful and swollen. Patients with complete tendon tears cannot stand, do a straight leg raise while lying on their back, or extend their knee while seated. Long-term complications (eg, loss of motion, weakness) are common.

Pathophysiology. Traumatic knee extensor injuries usually involve quadriceps contraction against a flexed knee, such as when a patient is attempting to regain balance after a fall, jumping, or missing a step on the stairs. Significant injuries may also occur in patients with comorbidities after a simple fall.

The extensor mechanism of the knee consists of the quadriceps muscle and tendon, the patella, the patellar tendon, and the tibial tubercle. Disruption of any of these can lead to an extensor mechanism rupture and render an otherwise perfectly good total knee replacement (TKR) useless.

Patella fractures are caused by direct trauma or compressive force, or as the indirect result of quadriceps contractions or excessive stress to the extensor mechanism. Indirect injuries are commonly associated with tears of the retinaculum and quadriceps muscles.

The extensor mechanism is responsible for extending the tibiofemoral joint. The term refers to the common linkage of the four converging quadriceps muscles into the quadriceps tendon attaching proximally to the patella bone, continued distally by the patella ligament or tendon to the tibial tuberosity.
