DNR is short for “do not resuscitate.” Resuscitation is when someone who has stopped breathing and whose heart has stopped beating is restored to consciousness.

However, only a physical, the patient, or the patient's healthcare agent can revoke a DNR. The only instance in which a family member can revoke a DNR is when that same family member is also the patient's healthcare agent.

Have the witnesses sign and date this and attach it to the DNR form. Does the form have to be notarized? A notary public can witness the signature of the person filling out the form in lieu of two witnesses. If two witnesses are present, the form does not need to be notarized.
