- Proceed to Section III, Authorized Signature. This section must be signed by an authorized representative of the entity. Enter their name, title, telephone number, email address, and fax number, ensuring all details are accurate before proceeding.
- Once all sections are completed, review the form for accuracy and completeness. Ensure that the total allocation matches the stated amount in Section I.
