- Line 7 requires the date of transfer. Fill in the exact date of the property transfer in MM-DD-YYYY format.
- In Line 8, check the box if the property is classified under specific programs that could incur penalties.
- In Line 9, indicate if there were any special circumstances affecting the sale price by checking the appropriate box and providing details.
- Line 10 involves income tax withholding. Indicate applicable exemptions if the buyer is not required to withhold income tax.
- Line 11 requires an oath, so input the preparer's name, mailing address, phone number, and email address.
