- Fill out Part V, the Seller’s Declaration, where the seller declares the sale details, including the actual purchase price. The seller must sign, print their name, and date the form.
- Compile the necessary supporting documentation including the sales receipt, proof of payment, and any explanations for the lower purchase price as described in the instructions.
- Review all information for accuracy. Save your changes, download a copy of the completed form, and print it for submission.
- Submit the completed CERT-106 and supporting documents to the Department of Revenue Services at the provided address within the three-year timeframe.
