- In the Services Selection section, fill out the services description, status, quantity, and fees. Make sure all financial figures are accurately calculated excluding VAT.
- Provide payment details in the Payment Details section. Indicate how you will be paying for once-off service fees and monthly service fees.
- Review the Acknowledgement and Agreement section. Sign and date the form, providing your full name, ID number, and capacity in which you are signing.
- Finally, make sure all required documents are attached, based on whether the customer is a natural or juristic person as specified.
